Practical support for adult life.

Explore practical assistance for young people transitioning from children’s services into adulthood. Individual eligibility, access needs, preferences, location and availability must be confirmed before support is agreed.

Understanding individual needs

Specialist support

Person-centred support for young people during transition, recognising communication preferences, learning needs, accessibility and wellbeing. Eligibility and suitable arrangements are discussed individually.

Autism support

Support around communication preferences, sensory needs and routines, as well as social participation, confidence and practical skills.

Learning disability support

Support for young people with learning disabilities to make choices, practise everyday skills and participate in activities during transition.

Learning difficulties support

Practical assistance with understanding information, organising tasks and learning everyday skills, shaped around the young person’s learning preferences. Learning difficulties are considered separately from learning disabilities.

Support with physical disabilities

Practical assistance with organising everyday routines, participating in community activities and accessing appointments, taking account of the young person’s accessibility requirements. Suitable arrangements must be discussed individually; this is not specialist physical therapy.

Support with mental health needs

Person-centred practical support with routines, participation and changes in everyday responsibilities, alongside involved professionals where appropriate. This is not clinical treatment, psychiatric care, counselling or rehabilitation.

Transition towards adulthood and adult social care

Support with independent living skills, travel confidence, education, training, volunteering, employment and decision making. We can explore changes in support alongside families and involved professionals where appropriate.

Familiar routines

Home & family support

Practical support with household tasks, meal preparation, companionship and independence skills for young people preparing for adulthood.

Family support

Work alongside young people and their families to understand routines and priorities, explore practical support and prepare for changes in services and responsibilities.

Personal assistant support

One-to-one practical assistance for a young person preparing for adulthood: organising daily routines, shopping, meal preparation, companionship and practising independence skills at home or in the community. Activities are agreed around the young person’s goals.

Sitting service

Companionship and agreed activities with a young person in familiar surroundings while a family member or carer takes time away. This may include hobbies, conversation and practising everyday skills. Timing and arrangements are discussed individually.

Respite support in the family home

Planned companionship, activities and practical assistance for a young person in the family home, giving families and carers an agreed break during transition. We discuss the activities, suitability and timing before agreeing arrangements.

Light household support

Help with light cleaning, tidying, laundry, ironing, changing bed linen, organising belongings and meal preparation where these form part of the young person’s agreed transition support.

Taking part

Community & independence

Opportunities to practise skills, meet people and take a more active part in everyday life.

Outreach support

Outreach support for young people moving towards adulthood, connecting everyday goals with activities and resources in the community. Individual suitability, access needs and arrangements are discussed before support is agreed.

Community access and social inclusion

Help to participate in activities, friendships, appointments and hobbies, and explore local community resources.

Independence and life skills development

Practice with shopping, meal planning, budgeting, travel, time management, decision making and self-advocacy, at a pace that works for the individual.

Volunteering and employment escorts

Practical accompaniment to volunteering or work, supporting travel confidence, familiar routines and participation. Arrangements are agreed around individual needs.

Everyday organisation

Practical support

A helping hand with the appointments and organisation that form part of everyday life.

Hospital and healthcare escorts

Practical assistance, reassurance and communication support during healthcare appointments. This service is accompaniment and support, rather than formal independent advocacy.

Administrative support

Help with forms, correspondence, appointments, organising records and finding relevant services. Specialist legal or benefits advice should be obtained from a qualified adviser.

Arranging support

Contact Timely with a general outline of the young person’s transition support requirements and the proposed location. We will discuss whether suitable support is available before any arrangements are agreed.
