Supporting young people through the transition to adulthood
About Timely

Supporting young people towards adulthood.

Timely Integrated Support Services Limited supports young people transitioning from children’s services into adulthood, including those with autism, learning disabilities, learning difficulties, physical disabilities and mental health needs.

Our purpose is to provide practical, person-centred support during the move towards adult life, working alongside young people, families and involved professionals where appropriate.

Our person-centred approach to transition

The move from children’s services into adult life can bring changes in education, support arrangements, relationships and everyday responsibilities.

We begin by understanding the individual’s strengths, communication preferences, sensory needs, routines and goals. Support should reflect the person’s choices and the tasks or activities they want help with.

We agree practical goals together, which may include:

  • Meal preparation, household routines and budgeting.
  • Communication preferences, choice and self-advocacy.
  • Community activities, relationships and travel skills.
  • Appointments, education, training, volunteering and work.

Activities are discussed around the young person’s needs, preferences and accessibility requirements.

Partnership with individuals and families

With the individual’s agreement, we involve families, carers and relevant professionals. Schools, colleges, healthcare and social care teams may contribute information that helps keep support coordinated, particularly when preparing for changes in support. We do not make statutory assessment or funding decisions, or guarantee continuity between public services.
